Job Summary/Basic Function
Supports implementation of the Babies To Be Program by delivering direct services to participants, supervising Social Work students, and executing outreach and engagement activities. The Program Coordinator manages participant caseloads, conducts intakes, delivers educational content, and supports day-to-day program functions under the coordination of the Program Manager and guidance of the Program Director.



Minimum Qualifications
  • Bachelor's degree from an accredited program with 2 years of related experience in case management, program coordination, maternal health, or community-based services.
  • Licensed as a Social Worker (LISW-S with supervisor designation) or the ability to acquire licensure in Ohio within 6 months of appointment or hold a Bachelor Science of Nursing and be a Registered Nurse or hold a Master's in Counseling.
  • Demonstrated ability to supervise.


Preferred Qualifications
  • Licensed in Ohio at the master's degree from an accredited program.
  • Familiarity with resources in Montgomery County for pregnant moms and their families.
  • Supervision experience and a team player


Essential Functions and percent of time
50% - Participant Services & Case Management
  • Conduct participant intakes and maintain assigned caseload
  • Provide ongoing case management and support to pregnant participants and families
  • Deliver social-emotional and maternal health education aligned with Babies To Be curriculum
  • Connect participants to community resources to address social determinants of health


25% - Program Implementation Support
  • Execute daily program activities in coordination with Program Manager
  • Maintain accurate participant records and enter data into tracking systems
  • Assist with lesson planning, scheduling, and session facilitation

15% - Community Outreach & Engagement
  • Participate in community events, health fairs, and outreach activities
  • Support recruitment and retention of program participants
  • Assist in distributing program materials and increasing program visibility

10% - Student Supervision & Training
  • Provide direct supervision and mentorship to Social Work students
  • Ensure students meet academic and training requirements
  • Support students in developing and delivering program lessons
  • Review and oversee student documentation and data entry
