Clinical Pharmacist Position Type: Unclassified Department: NEOvations Pharmacy Services Full or Part Time: Part Time Pay Grade: BW14 Summary:
The Clinical Pharmacist is a part-time ambulatory care position integrated in a primary care practice which is responsible for the execution of medication therapy management ( MTM ), disease state management through collaborative practice agreements ( CPA ) and provision of immunization services. Position involves precepting and supervision of pharmacy interns, residents, and/or other employees.
Principal Functional Responsibilities:
Pharmacy Services: Deliver telehealth and in-person pharmacy services, including but not limited to:
Comprehensive Medication Reviews ( CMR )
Targeted Medication Reviews ( TMR )
Medication Reconciliations
Disease management through CPA
Substance use disorder treatment
Travel health consults
Clinic Services: Manage and expand immunization...
