Community Health Worker - Score 1 for Health R-0002773 Joplin Campus Job Type: Staff Job Description: The Community Health Worker will primarily be working with families and school partners from the Joplin community. Additionally, this position will work closely with the whole Score 1 team, Joplin metro medical providers, vision providers, dental providers, and other agencies to improve access to primary, medical, dental, and vision care for Score 1 for Health children. Essential Duties and Responsibilities:
Convey the purpose and service of program to families and the impact that program or service would have.
