Clinical Assistant - Diabetes Care Clinics Primary Purpose: The Diabetes Patient Care Clinics, funded by Abbott and administered through the Thomas J. Long School of Pharmacy, provide year-round, direct patient care services to the Stockton community. The Clinics focus on screening, prevention and management of diabetes, targeting individuals who are at risk for, or diagnosed with, prediabetes or diabetes. Services include health screenings and testing, patient-centered clinical services, disease-state education, medication management, and strategies to optimize medication access and affordability. The Thomas J. Long School of Pharmacy is committed to preparing students for lifelong success in health-related careers through an exceptional, student-centered learning environment.
